Physician-led hormone therapy, GLP-1 weight loss and peptides, prescribed off your own lab work.
Hormones, weight loss and peptide care with a local clinical team. Every program starts with an assessment and a physician who reads your results.
We help you understand your options. Your physician decides what is appropriate after assessment and labs, and every protocol is written only after your provider reviews labs.

Coming to the patient portal
Drag the line to compare. Enrolled ELEVATED GLP-1 patients will be able to see a private preview like this one, made from their own photo, once their provider turns it on. Your photo stays private in your portal and is never used in marketing.

Peptide therapy
Recovery, body composition, longevity and skin protocols from licensed compounding pharmacies. Every one starts with an assessment and a provider, and none of them is sold off a shelf.

Nurse practitioner
Wellness assessments · Hormone & weight management · Lab review · Follow-up visits
Madison is a family nurse practitioner. She sees patients for Wellness Assessments and follow-up visits, reviews lab work, and manages ongoing treatment plans, including hormone therapy, weight management, and peptide protocols, under our physicians' protocols.
Her background is emergency nursing, which is the same place this clinic started: seeing how much of what arrives as a crisis was preventable years earlier.

Your clinical team
Intake · Member visits · Care coordination
Caroline is who you see for intake, member check-ins and follow-up coordination. She operates under physician-authorized standing orders with direct access to our physicians when your case needs physician review.
A familiar face when you visit and a clinical team that actually knows your chart, not a rotating telehealth queue.

It is an in-person visit with our clinical team in Evans. We review your goals, history and vitals. When labs are indicated, your provider supplies an order for an outside laboratory; fees are confirmed before testing. The visit is how we decide whether an ELEVATED program is right for you. If you enroll, the $79 is credited toward your first program invoice.
